I hereby grant permission Adams Landscape Management's insurance agency to secure my Motor Vehicle Report (MVR) to determine my "driving insurability" under the company's automobile policy and to release this report to my employer/prospective employer. I understand that I will not be authorized to drive a vehicle on company business until such time that my MVR has been approved. I also understand that if 1 am employed to drive company vehicles and it is determined that my MVR is not approved for "driving insurability" under the company's automobile policy, my employment may be terminated.
